      If you could reimagine your diabetes technology, what’s the one thing you would change?
      The technology is remarkable — and I’m thankful for it. Having managed T1D for a very long time, it's improved my A1C. But as we age with T1D, usability becomes critical. Larger fonts, easier interfaces, simpler navigation, and design for arthritic hands will matter more and more. We also urgently need better training in hospitals and care facilities. Too often staff are unfamiliar with pumps and CGMs, and patients are forced to disconnect from the very tools that keep them safe. With the nationwide shortage of endocrinologists, we cannot rely on specialists to fix these gaps — frontline medical staff need better training and support. Tech innovation must include accessibility and real-world medical training.

      An atrophied imagination is the bane of progress in any subject: theology, economics, science, art, medicine, whatever. So, with my myopic and very limited Lilliputian understanding, I would prefer advancement in sub-cellular or cytoplastic or the rewiring of six of the primary enzymes of the pancreas but particularly the beta cells. Particularly plenipotentiary stem cells that can crank up the enervated beta cells. ꧁⎝ 𓆩༺✧༻𓆪 ⎠꧂

      and that would also save us the 30 minute phone call where they make you feel like you did something wrong and they may deny you a replacement for their product that failed.. again!

      Automatic coverage, no questions asked for replacements. (until they discover that cure that's always 5 years away) Seriously, I'll deal with the failing sensors, the clogging pods and whatever else if I know I can just reorder and get them in a timely manner - avoiding that panic attack that happens every time something fails on me.

      On my Tandem Pump, make the numbers larger, including the Insulin On Board. Stop using "red" colored numbers, and make them larger. I cannot focus my eyes on red lights. Have the Control IQ do corrections more often, so blood glucose does not go so high. Make the tubing on the VARISOFT, 13mm infusion sets 43 inches, again. Thirty-two inches is too short. Make the Overpatches easier to handle so they don't stick to my hands and go on all wrinkled. Oh, I'm sorry. You said one thing. Oops.

      Is this a "DOH!" question? Anyone who's read the 1993 DCCT results knows the compelling message that CV effects are killing most diabetics, not neuropathy, nephropathy, and retinopathy. ˋ°•*⁀➷

      Every organism is metabolically programmed to prioritize its own survival, by working to create order out of nature’s chaos. Humans require a lot of skull sweat to seek the Edenic solidity of St. Certainsville. And—as Jacques Derrida might have said—a kind of pharmakon: a Greek word that the philosopher noted could be translated as either “remedy” or “poison,” depending on your perspective. A major criticism of AI? What unsettles users is not just that the system lacks beliefs but that it keeps apologizing as if it had any. The potential for AI in biology, the Genome project, unfolding protein molecules at the atomic and subatomic level is phenomenal. Whether it cures us or kills us is, well, pharmakon.

      Like the Industrial revolution and the Information Age, it’s the next big revolution. It will impact everything and hopefully help human beings, who are so overwhelmed with data now, better utilize, store, and manage the data.

      I think AI has the potential to change everything that involves data which just about everything, whether factual or opinion. As data storage shrinks or connectivity rises it will become even more so capable. For example, there an old StarTrek episode where Dr Daystrom bases a new computer storage medium and process on his own brain engrams. The episode points to the advances and pitfalls of the potential.

      It may be involved. But man in the loop is required to lead it, feed it and control it. There is no common sense associated with it as it is just another tool to help man.

      for Tandem IQ, i would love if it wouldn't react to a 79 blood sugar so dramatically. stopping all insulin.. then my sugar bounces up to 150-180 for an hour after. i hear that the new sequel twiist gives more control, but it's not available in my pharmacy for coverage yet.

      AI lends itself to find BG patterns and could be taught to adjust basal rates.

    Samantha Walsh has lived with type 1 diabetes for over five years since 2017. After her T1D diagnosis, she was eager to give back to the diabetes community. She is the Community and Partner Manager for T1D Exchange and helps to manage the Online Community and recruit for the T1D Exchange Registry. Prior to T1D Exchange, Samantha fundraised at Joslin Diabetes Center. She graduated from the University of Massachusetts with a Bachelors degree in sociology and early childhood education.

      No.

      But I wonder if some physicians don’t sort of group patients into the 3 parts of the bell shaped curve: 1) this guy’s got it down pat (17%), 2) the majority can do a little better (66%), and 3) this one doesn’t have a prayer (17%).

      My long-term/short-term goal is to survive another 6 months by continuing to have access to insulin and our CGM (or an adequate quantity of teststrips) and to avoid things that might jeopardize my survival or make things worse.

      I said, “not sure”, The discussion of goals is a vague concept. It’s more about discussing blood test results, A1c, how am I feeling. Sometimes we may reference previous problems, and are they still a problem. I think that is more often brought up by me.

      My numbers are pretty steady every three months. Except for my TSH. Thyroid still out of range even on medication. Was a little concerned about A1c because I just had steroid therapy and also went to Italy but was pleased it was 5.6. Being a little OCD can be a good thing.

      My provider is part of an internal medicine practice so we discuss a wide range of health issues as needed (in addition to diabetes concerns).

      The “goal” is relative. Range is now the focus as well as overall health, mood, vital signs, kidney function and weight. My team is focused on minimizing lows which not surprisingly results in realistic ranges and quality of life. Careful not to let the goals blind us to how we actually feel. Call it existential care. Hope all are well. You are an extraordinary group of folks managing an insidious disease with courage, grace and fellowship.

      Sometimes they bring it up, sometimes I need to. Nowadays it is all documented in the electronic records, although not always accurately (which is sometimes why I need to bring it up to get it discussed).

      My diabetes “goals” are A1c below 7 and >70% TIR. I’ve hit those each appointment for the past 4-5 years. What other goals might someone have?

      I bring up ideas and he’ll generally agree but than again I’m sure like most “here” I’m proactive and read a lot on diabetes.

      But… as a general practitioner he sends all his TYPE 1 to a clinic, and his “out of control” TYPE2. He compliments on my control and improvements (last two a1C were 6.3 and 6.0 (again I’ll admit I’m his only patient he sees as a TYPE 1, and using a pump/CGM.
