Background

Widespread Inequities in outcomes among people living with diabetes have been widely published. Those from minority communities, of low socio-economic status, publicly insured, of Black race, Hispanic ethnicity, and those living in rural areas, are less likely to be on diabetes technologies and unlikely to achieve optimal glycemic targets. There are few resources on practical tools and successful initiatives by diabetes care and education specialists to reduce gaps in health outcomes. This session will share a real-world, successful 10-step practical framework employed by the T1D Exchange Clinic network of 60 diabetes centers to reduce equity gaps and improve overall outcomes.
